Heparin Over-Dosage Due to Similar Containers

Overdosages of any medicine can be deadly. In the recent case of Dennis Quaid and his wife Kimberely Quaid, the medication dosage error almost cost them the lives of their twins. Heparin Overdosage News - Dennis and Kimberly Quaid Sue Drug Company Baxter

Dennis Quaid and his wife Kimberly Quaid have sued Baxter, manufacturer of the drug Heparin, claiming that Baxter Healthcare Corporation, makers of the anti-coagulant Heparin, failed to properly label vials of its product and that the 10-units-per-milliliter vial and the 10,000-units-per-milliliter vial are virtually identical. The Heparin lawsuit by the Quaids allege that Baxter knew of previous dosage mix-ups, yet failed to recall shipments of the drug or properly warn hospitals of the dangers. The Quaids twins were mistakenly given 1000 times the dosage of Heparin on Novemeber 18, 2007 resulting in serious and potentially life threatening injuries.

FDA Notice - Heparin Dosages and Potential Medical Errors

February 7, 2007 - Baxter and FDA notified healthcare professionals of the potential for life threatening medication errors involving two Heparin products, Heparin Sodium Injection 10,000 units/mL, and HEP-LOCK U/P 10 units/mL. Baxter is aware of fatal medication errors that have occurred when two Heparin products with shades of blue labeling were mistaken for each other. Three infant deaths resulted when the higher dosage Heparin Sodium Injection 10,000 units/mL was inadvertently administered instead of the lower dosage of HEP-LOCK U/P 10 units/mL. The currently marketed 1 mL vials of both Heparin products use blue as the prominent background color on their labels.
